General Information

HVGRC's All-Breed Super Health Clinic will be held on Sunday, January 31, 2010 at the Norwalk Veterinary Hospital from 8 AM to 4 PM.

Pre-registration is required.

Xylazine only.  Dogs weighing more than 80 pounds $165.  No dogs over 100 pounds
*Hips and Elbows Dr. Charles Duffy $180
$25 additional disc
Xylazine only.  Dogs weighing more than 80 pounds $225.  No dogs over 100 pounds
*OFA hips/elbows are now digital.  Consequently, all will be submitted to OFA.
* Copies of the disc will be available for $25.  Include this fee plus the applicable OFA fee in your check payable to HVGRC.  Bring a stamped, self-addressed envelope to the clinic.
Eyes Dr. Charles Stuhr $38 per dogBoard-certified ophthalmologist. CERF submission by owner. CERF fees not included.
Hearts Dr. Jean-Paul Petrie $40 per dog Auscultation only. Board certified . OFA submission by owner. OFA fees not included.  Doppler available ($250)
Thyroid OFA Panel $85Blood to be submitted to Michigan State University. Cost includes OFA fee
Microchip   $40 per dogHome Again microchips.

You may request a preferred appointment time in either the morning (8 AM-12 PM) or afternoon (12 PM - 4 PM) but appointment times cannot be guaranteed; requests will be accomodated to the extent possible.

Confirmations will be mailed approximately 10 days prior to the clinic date.

Remember to bring your AKC registration papers and tattoo/microchip number with you for OFA and CERF submissions.

Cancellation policy: prior to deadline, $20 fee; after deadline, 80% refund.

From I-95 North or South Exit 13 off I-95. Take a right at end of ramp onto Connecticut Avenue (also labelled US 1). The hospital is less than a quarter mile on your left between McDonald's and the Shell station.
From US Route 7 South Take the "Connecticut Avenue / US 1" exit on Route 7. Head west approximately one mile. The hospital is on your right between McDonald's and the Shell station.
